Put the decisions in one document
A useful specification names the product, materials, dimensions, construction, colours and finishes, branding, labelling, packaging, estimated quantity, destination market, and any testing that market requires.
If a decision is still open, mark it as open. An open point can be sampled. A hidden assumption usually shows up after production has started.
